We respect your personal privacy and will not use your personal information in any way that we would not accept ourselves. We have set out the following information about how we manage the personal information we do collect. 1.WHO WE ARE

We are the Jade Screen Project (“Jade Screen”), a coalition of experienced practitioners and researchers of Chinese herbal medicine. We provide an online portal (“the Portal”) through which front-line workers in the COVID-19 pandemic can apply for consultation with a professional herbalist

3. THE INFORMATION THAT WE MAY HOLD

We hold the following different types of personal information

3) Medical information – so that we can process the inquiry appropriately
Why we hold it1) We hold this information so that we can handle each request for assistance individually
2) We may then keep this information for our records and for analytics
Why we are allowed to use the information1) Consent – we do not obtain information unless the inquirer has consented to share their information
Who we will share the information with1) we will share information with members of the Jade Screen coalition
2) IT hosting company: We use a company called SiteGround that provides us with IT services such as hosting, database and file storage
How long we will hold the information forWe will keep the information for 2 years from the date it is supplied
How we obtain the informationThe information is supplied by inquirers via our website form

4. YOUR RIGHTS

If we hold information about you then you have the following rights in respect of our use of that information:

1.A right of access to your information and to details about how we use your information

2. A right to rectification of inaccurate information

3. A right to have information erased (a right to be forgotten)

4. A right to restrict the use we can make of your information

5. A right to information portability

6. A right to object to the processing of your information

We will comply with any of these rights as soon as possible.

We will not charge for any request unless it is manifestly unfounded or excessive (for example because it is repetitive). If a request is manifestly unfounded or excessive then we will either charge our reasonable administrative costs to comply with it, or we tell you that we are not going to comply because it is manifestly unfounded or excessive.
